To register for lessons, you can either contact our office or fill out our registration forms below. You'll find all of the enrollment forms needed for the particular Driver Ed Academy program you have signed up for.
*If you completed your classroom online and need the 8 hours of behind the wheel training, see the registration forms below.
Complete your registration
Once you have completed your 24 hours of classroom and have your Certificate of Completion, print the appropriate forms (found below) and email them to: DRIVEREDACADEMY@GMAIL.COM

Forms for: 8 hours behind the wheel training for ClassD

Taking the online classroom and you need to enroll for your 8 hours of behind the wheel training:
Print out the following forms located below. All documents will need to be sent in document form. (Scanning is best) No pictures will be accepted (except your permit). We will pick you up for all lessons. There are 4 lessons, 2 hours each.

We will need the following items from you:
- Copy of your temporary permit
2. Payment of $649.00 ($629.00 with Costech Technologies Certificate)
(check, cash, MO) (2% fee for credit or debit card)
3. Original completion certificate from online classroom
4. One each of the following forms below, signed and dated

- Record of Agreement (HAMILTON, CLERMONT)
- Emergency Medical Form
- Code of Conduct for Behind-the-wheel
